19/2018 : Corp_New Group Insurance Card

เนื่องจากประกันกลุ่มของบริษัท ที่ได้ทำไว้กับ บมจ. อลิอันซ์ อยุธยา ประกันชีวิต (AZAY) กำลังจะหมดอายุลงในวันที่ 7 สิงหาคม 2561 นั้น ทางบริษัท ได้ดำเนินการคัดสรรบริษัทประกันใหม่เพื่อเข้ามาดูแลผลประโยชน์และให้ความคุ้มครองแก่พนักงานในกรณีการประกันชีวิต ประกันอุบัติเหตุ และกรณีการรักษาพยาบาลแบบผู้ป่วยใน โดยคำนึงถึงผลประโยชน์และความเหมาะสมของทั้งพนักงานและของบริษัท โดยบริษัท ได้คัดเลือก บมจ. โตเกียวมารีนประกันชีวิต (ประเทศไทย) มาดูแลผลประโยชน์อย่างต่อเนื่องให้กับพนักงาน โดยมีผลความคุ้มครองตั้งแต่วันที่ 8 สิงหาคม 2561 เป็นต้นไป

ท่านสามารถตรวจสอบวงเงินประกันได้จากไฟล์ที่แนบมาด้วย หรือ ดาวน์โหลดแอพพลิเคชั่นทางโทรศัพท์มือถือได้ตาม QR Code ด้านล่างเพื่อความสะดวกของท่าน ทางบริษัทจะจัดส่งบัตรประกันกลุ่มใบใหม่ให้แก่พนักงานตามไซต์งาน พร้อมด้วยใบคำขอเอาประกัน หรือสามารถมารับได้ที่ฝ่าย Corporate Servicesได้ตั้งแต่วันนี้เป็นต้นไป โดยทางพนักงานจะต้องเตรียมสำเนาบัตรประชาชนของตนเองมาด้วยเพื่อส่งมอบให้กับทางบริษัทประกันพร้อมใบคำขอเอาประกัน

*เฉพาะพนักงานที่ผ่านการทดลองก่อนก่อนวันที่ 2 สิงหาคม 2561 เท่านั้น จึงจะได้รับบัตรประกันในรอบการจัดส่งนี้

Since the group insurance which is supplied by Allianz Ayudhaya Insurance (AZAY) is going to be expired on 7 August 2018, the company has selected the new insurance company by considering appropriate employees’ and company’s benefits. This new insurance company, which is Tokyo Marine Life Insurance (Thailand), will continuously take care of employees’ benefit and coverage for life, accidents, permanent disability, in-patient hospitalization, and surgery. This benefit will be effective on 8^th August 2018 onward.

You may check insurance benefits and coverage from the attached file or download a mobile application from QR code below for your convenience. The company is going to distribute the new cards with insurance application forms to clients’ sites. Or you may pick up the new cards at Corporate Services department by preparing your copy of ID cards to submit to insurance company.

*Please note that only employees who passed probation before 2 August 2018 are going to receive new insurance cards with this distribution.
