ทางบริษัทฯ มีความห่วงใยในสุขภาพของพนักงานทุกท่าน จึงจัดให้มีบริการตรวจสุขภาพประจำปีและการฉีดวัคซีนป้องกันไข้หวัดใหญ่ โดยพนักงานสามารถเข้ารับบริการได้ที่ โรงพยาบาลเปาโลเมมโมเรียล สะพานควาย ทุกวันเสาร์-อาทิตย์ ระหว่างวันที่ 1 กันยายน 2561 ถึง 28 ตุลาคม 2561 และเพื่อให้การเตรียมตัวเป็นไปอย่างถูกต้อง ทาง Corporate Services ขอให้ทุกท่านศึกษาสิทธิ์ของตนเองและการเตรียมตัวก่อนเข้ารับการตรวจร่างกายตามรายละเอียดในลิงค์ หรือข้อมูลโดยสรุปด้านล่าง

1. ควรไปรับการตรวจในเวลาเช้า และนำบัตรประจำตัวประชาชนไปด้วย

2. ติดต่อ ศูนย์ตรวจสุขภาพ อาคาร 1 ชั้น 13

3. ทางเวชระเบียนจะขอทำสำเนาบัตรประชาชนไว้เพื่อประกอบการส่งอินวอยซ์

4. ให้พนักงานระบุสิทธิที่ตนเองได้รับลงในสำเนาบัตรประชาชน

5. งดน้ำและอาหารเป็นเวลา 8-10 ชม. ก่อนเข้ารับการตรวจ

6. งดเครื่องดื่มแอลกอฮอล์อย่างน้อย 24 ชม. ก่อนเข้ารับการตรวจ

7. แจ้งให้แพทย์ทราบหากรับประทานยาชนิดใดต่อเนื่องหรือมีโรคประจำตัว

8. งดใส่เครื่องประดับที่เป็นโลหะ

9. สตรีไม่ควรเข้ารับการตรวจในช่วงมีประจำเดือน และระยะ 7 วันก่อนและหลังมีประจำเดือน

10. กรณีสงสัยว่าตั้งครรภ์ โปรดแจ้งเจ้าหน้าที่ให้ทราบ

**ทางบริษัทขอความร่วมมือพนักงานใช้สิทธินี้ภายในระยะเวลาที่กำหนด เนื่องจากเป็นเงื่อนไขของทางโรงพยาบาลค่ะ

Dear all BE8 Family

The company is concerned about employees' wellness and healthiness; thus, we are arranging Annual Health Check and Influenza vaccination (seasonal Flu vaccine). Employees can use this service at Paolo Memorial Hospital Saphan Khwai on every weekend from 1 September 2018 to 28 October 2018.

To correctly prepare yourselves, Corporate Services team would like you all to take a look at the preparation below or from the link below.

· Please have the service in the morning and bring along your ID card/ passport

· Contact Health check centre building 1, 13th floor

· Medical record department will photocopy your ID card/ passport

· Please specify the service that you receive (health check, Vaccination)

· 8-10 hours fasting before the physical

· Please do not consume alcohol within 24 hours before the physical

· list of current medications you take, including over-the-counter drugs and any herbal supplements or inform the doctor if you have any chronical health conditions

· Do not wear metal accessories

· For women, please do not have the physical during your period, or 7 days before and after period

· Please inform the doctor if you suspect pregnancy

** We would like to ask for your cooperation in completing this service within mentioned date and time due to the hospital’s restriction.
